Ice and water membrane goes around all four sides regardless of whether the kit includes it, because the kit assumes a climate with less freeze-thaw than the one this valley delivers between December and March.
If yours is being replaced during a re-roof, replace the unit rather than refitting it. A twenty-year-old skylight set back into a new roof will outlive its seals long before the covering does, and reopening a finished roof to change it three winters later costs a great deal more than the unit.
